Step-by-Step Guide

Schedule an Appointment

Contact our staff to set up an appointment. We will ask you about your basic information to start your profile and send you the initial information you need to come prepared for your first appointment.

You will be contacted by our staff after the initial contact to confirm your appointment prior to your visit.

Prerequisites

To facilitate your appointment and make it a smooth first visit at The Spine Care Center, our staff will ask you to fill certain forms. Depending on which ones you are asked to fill, you can choose from the options below, download and print the form, fill it with the pertinent information, and bring it with you the day of your appointment.

You will also have to complete the following form needed to become a new patient:

For your first appointment, be sure to bring:

  • Your completed Patient Form and Patient Demographic Information
  • A valid driver’s license or equivalent identification document
  • Your health insurance card or information
  • A list of all the medications you are currently taking, their prescribed dosage, frequency and reasoning behind each medication
  • Bring copies of your previous studies to the related issue or other medical conditions. In these copies, you can include x-rays, CT or CTA, MRI images, reports, injection history, and your medical records for previous related conditions and treatments. If you have any copies of this information in a CD or Video/Film, please bring them with you since we can’t access any information that lives outside of our facility.

Initial Assessment

Upon arrival at your first appointment, you will be received by one of our medical assistants to take you to the examination room. General information such as weight, height, and pictures, will be taken by the assistant taking care of you to add to the new medical profile — some of these will be taken every visit to ensure a track record of your current situation.

If you were asked to bring any information or documentation, this would be the time for the assistant to receive that information.

Examination

At this point, you will be receiving a more detailed physical examination from one of our Physician Assistants who will be asking you questions about your medical history and reviewing in-depth the documentation and possible imagery that you might have been asked to bring. This would also be the right time to perform any new x-rays to have more information in order to educate you about your condition.

Diagnostics

Once all the information has been taken into consideration, your Physician Assistant and your Spinal Specialist will work together with you to give you all the information you need to know about your current condition, they will spend the time with you to answer any questions you may have about what they have diagnosed and the future of your treatment or steps to take to help you improve on your situation.

All our deductions and plans of action are done with you, the patient, in mind. Everything we do is to make you improve and feel better and all of our decisions are made to satisfy your needs and make you feel better.

Final Step

After the assessments, conversation, diagnosis, and conclusion, you will receive all the documentation that you need about what was done that day. You will also receive any necessary prescriptions.

Your assistant will help you with scheduling your future appointments or follow-ups and accompany you to the patient care desk to receive your finalized paperwork.
